Making and Registering a Representation Agreement (Part 2)

Ready for action? Get your legal document in place BEFORE a health crisis! Learn how to complete a legally valid Representation Agreement form as available on the Nidus website. Who can be your witnesses? Who else has to sign? Find out how to register the completed document with the Personal Planning Registry.

You can purchase a hard copy of the Representation Agreement form and instructions at this presentation (optional). The fee is $7.00 for individuals or $10.00 for spouses. 

It will be beneficial to attend Part 1 (above) – The Essentials of a Basic Plan for Your Future. There are other options for personal help if you cannot attend this presentation. They will be discussed at Part 1 or you can email info@nidus.ca
